 can anyone help?

i don't really know where else to post this so if anyone can give me some information i'd really appreciate it!
here's my question:
my fiance has an "outie" belly button. he always complains that whenever it is touched, it sends a pain through his body so it is very tender. he also says that he trys to clean around it and when he does, there is a horrible smell and it causes him severe pain. does anyone know what could be causing this or been in a similar situation before?
