Ok, this has been going on for almost 2 months now. Here's my story:
I'm gay, had unprotected sex with a man before xmas and i was the top. 1 week later i had discharge. Got home from xmas and went to the doc and she took a look at it and gave me 1 dose of cipro and 20 doses of doxy. The symptoms got better, but not 100%. A week after the medication was over with, the symptoms came back. Urination was PAINFUL, i could barely pee. I didn't want to pee it hurt so bad. I went back to the doc and she put me on 1 week of metronizadole (for a possible parasite) and 2 weeks of cipro (in case the first dose wasn't enough). It did the trick and the symptoms (of that!) are gone. I'm guessing it was gonorrhea which just wasn't killed the first time by the single dose of cipro.
But now I have this shiny, scaly penis head that wasn't there before. And it's only on my penis head. It doesn't itch, it doesn't blister or scab over or any of that stuff. It's just a consistant blotchiness that gets worse with masturbation, and gets better after. It never fully goes away tho...the redness....the discoloration. Plus the skin is shiny and scaly. It doesn't feel right and the skin looks thin and wrinkled sometimes. I tried a 1% hydrocortisone for a couple days and it did nothing, so i stopped.
i'm not sure what it is:
herpes? No blistering, no pus, no itching, no scabbing, and it constantly stays. Doesn't really go away.
eczema? No itching, and hydrocortizone doesn't work.
psoriasis? Again, no itching, and hydrocortizone doesn't work.
there is a slight flaking when the penis is erect, but not enough to confirm a "flaky penis head" and it doesn't itch.
The redness goes away (but not 100%) after 30 minutes or so. The scaly stuff has me worried tho. And it's only on the penis head, no where else.

everything i research: eczema, psoriasis, lichen planus, genital herpes.....they all say itching, but this doesn't itch at all. When erect, the skin is shiny and smooth, and when not erect, it's patchy-red, but no blisters or puss of any kind. And no itching.
And the redness fades away after an hour or so, but not 100%. It's almost like the blood vessels underneath the skin are causing the rash, tho it's not a blister.
i'm beginning to think it's just the way it is and i have to accept it. I mean...it doesn't bother me in any way, doesn't itch, or anything. It just gets blotchy after masturbation or sex and then goes away in an hour or so. Is this common?

I have a similar looking thing on the head of my penis. I showed it to my GP who said he didn't think it was anything to be concerned about but has referred me to a urologist.

I would have guessed eczema or psoriasis.
I think you have stumped both us and the doctor.
You get an F in safe sex, but an A in net research.
Perhaps it is time to see another dermatologist.

well this is still going on. And i know for a fact, judging on research on the net, that I am not alone when it comes to this. There seems to be no tell-tale sign of a probable cause, so i hope this gets some more responses. If i do a google search, i can come up with tons of relatable stories, but none of their questions have been answered.
I have boiled it down to PPP. I know, by looking at the base of my penis, that there is some PPP developing. That's ok. But does it affect the entire glans? My glans is wrinkled, is sometimes red and blotchy (not always), and it is noticeably different after masturbation or sex. But ultimately, it can change any minute at any time.
I can look at my glans and it can be blotchy without reason, and less than a minute later, it can look fine. That's how vastly different it can change. But it is wrinkly all the time. I had acquired chlymidia/gonnorhea before this, and i am 100% certain this has occurred after the fact after treatement. Maybe that's a culprit of the antibiotics, i'll never know. All i know is that i have a wrinkled glans, which sometimes gets blotchy with or without friction, and it occurs randomly, changing every minute of every day. It hasn't stayed smooth and shiny since. I know this isn't herpes. There isn't any irritation, no pussing, no scabbing, no itching, it doesn't bother me one bit. It just looks bad at times. And i don't think a creme would help, as it seems under the skin, it's not a topical issue. I know there are people searching for answers, as i've seen them. I just wish there was some clarity out there as to what this is. Hydrocortizone doesn't do anything, and it doesn't look like any pictures of psorisis or eczema or lichen planus. It seems like PPP, but all over the glans. Does PPP cause red blotchiness on the glans?
plus 1300 people have viewed this thread, so i know there are people with issues regarding this. It would be an ultimate PLUS if we could post pics, but sadly this is not the case. Please help us!
Hey Mateo, thanks for posting here buddy, you are definitely not alone in this.
I have a VERY similar experience, except I practiced unsafe sex with a woman. Got chlamydia, was treated, found out she also had herpes, tested negative for that, but ever since the treatment for chlamydia, my penis has been very different.
More sensitive, blotchy and rough at times with new papules I never noticed before, dry skin and irritation that can change on the dime. Super increased sensitivity, painfully so. So no, you aren't alone, but I can't afford a dermatologist so I'm just dealing with this until I get back from a trip I have planned.
Either way, keep looking into it, and if a dermatologist confirms what it is, please post it here so the rest of us have a base to go from when we all see doctors.
Oh my god I have the same problem for 2 years now. I am 24 years old. It doens't hurt or itch or anything. The head of my penis is just scaly and red. I'm not circumsized. I've been to 2 doctors and they say its yeast. At first it was just redness. I used antifungals and it went away. Then came back. Used antifungals again. Then came back again. and won't go away now. I used nystatin and most recently ciclopirox. NOThinG works. I am giong nuts trying to figure this out. I'm so depressed because of it. I've been keeping my foreskin peeled back for like 2 years now. When i let the foreskin go over the head, it gets sticky. Please help someone.
If it really is yeast, it's time to take a dietary approach, and eliminate everything in your diet which could be causing the yeast. There are plenty of sites out there which will tell you how. If it was yeast, your foreskin would have tightened from scarring by now.
I would see a dermatologist. Urologists know the plumbing, but dermatologists know the skin. I would bet it is eczema or psoriasis.
