Itching and foul smelling testicles, help please

Ok, for the past few months (I'm not entirely sure when it started) my scrotum and the skin on my thighs next to my scrotum have been red and itchy and had a somewhat scaly texture, sometimes they'd get so itchy that I would have to scratch, I couldn't actually stop myself. I was hoping this condition would go away but lately it's gotten worse. In the past few weeks my scrotum has started to smell and produce a sweat like substance (it's like sweat except it never goes away and it leaves white stains in my underwear where my scrotum is near the fabric, the stain is white but the liquid isn't) I haven't seen a doctor or a dermatologist yet as I'm rather embarrassed and I was hoping I could get a good answer here. I'm 18 and I'm a virgin and I refrain from using public restrooms so I don't know where this condition came from. I've seen a few posts about this before but none with a proper answer, I'm hoping that someone will know something now. Any help would be greatly appreciated.†

Dan, this sounds like a fungal infection. You could try an anti-fungal cream containing clotrimazole (such as Clonea or Canesten). You don't need a prescription; they are sold at supermarkets and pharmacies.

You might need a stronger medication. There is a pharmacy-only cream sold in Australia called Hydrozole. Again, you don't need a prescription but you have to ask for the cream at the counter. It contains hydrocortisone, which needs to be treated carefully.

Before applying any cream, wash your scrotum and dry it gently with a towel, then dry the area again using tissues. Make sure the area is thoroughly dry. Fungal infections thrive on moist, warm skin, so free-balling would be a good idea. Use a clean towel each time you shower and make sure your towels have at least four hours of sunlight when laundered.

Whatever you use, read and follow the instructions carefully. It would be a good idea to consult a doctor about using Hydrozole.

Thanks, I guess I'll have to try one of those creams, I don't want to see a doctor about it but if the creams don't work I will have too. Can a fungal infection lead to any health complications?

Hey Dan.

Fungal infections seem to be isolated to the area where they are active unless there is some transfer to other parts of the body (which is why I made suggestions about towels). I am sure they do not lead to wider health complications. However, as you have discovered, if they are ignored they don't go away. In fact, they get worse. Just like any problem, I guess.

You must apply either clotrimazole or hydrocortisone. If it was me, I'd be using hydrocortisone for the recommended seven days (and definitely no more than seven days), then I would use clotrimazole for at least a month even if the infection appears to have disappeared before then. Make sure you follow my suggestions about washing and drying your equipment thoroughly before applying the cream.

These creams will work. It takes time. If you use hydrocortisone first, you might see some improvement within a week. Your infection is in an advanced stage, so improvement will no doubt be slow. If you start using one of those creams now, you should not need to see a doctor.

Don't delay any longer. And please do not scratch that area. If you do, the fungal infection will be under your fingernails and I reckon you understand that it will be transferred elsewhere as a result.

Now you've got me scarred with saying I can't delay longer. Also what's the difference between a fungal infection and a yeast infection? Because I'm not entirely sure which I have.

Edit: also it looks like it's getting slightly better in some areas but worse in others

There is no need to be scared, Dan. These infections occur all the time, particularly with our climate, and they are easily dealt with. But you have delayed long enough, right? You have a fungal infection.

I can understand your embarrassment. But if you do nothing about the fungal infection, do you think it will go away?

Your mum and her boyfriend will understand the situation - if they find out. They don't have to. In any case, you need to deal with it.

I understand how this thoughtless lack of sympathy can be hurtful. If that is the way he treats you, your mother will see it sooner or later.

Do not allow the stupidity of others to prevent you from looking after yourself. Take action to get rid of the fungal infection and you will show your mother and her boyfriend that you can act independently.

Don't just stick hydrocortisone on it - thats very bad advice. Its a steroid and if you have a fungal infection and put that on its own it will make things worse.

The best thing you can do is go and see that doctor. This forum has lots of threads where people have ended up with a long-term scrotal problem that they've been unable to get rid of. Some of them blame putting steroids on. You don't want to end up in that club. Don't guess and try and self treat. Put embarrassment aside and go and see that doctor.

Dan, the advice I have given you is reasonable, sound and based on my personal experience of both clotrimazole and hydrocortisone following advice from my doctor, although I have not used hydrocortisone on my scrotum. Of course, any medication should be used strictly according to the instructions that come with the product. If I say any more I will just be repeating myself, so re-read what I have already written.

If you are in any doubt about what to do, seek the advice of a doctor. You can visit your nearest public hospital free of charge and ask to see a doctor about a personal problem. No one else needs to know about it. The doctor will respect your privacy and treat you with courtesy. Doctors have seen it all many times before.

The worst thing you can do is to ignore the fungal infection. Make a decision soon. I wish you all the best, mate.

Both of us I think are telling you to go and see and the doctor. They'll probably just need to take one quick look at it and then be able to tell you the right medication to clear it. Don't place embarrassment above your health. On a scale of things your health is way way more important. Or risk putting the wrong thing on when you don't have to.
