ok im a 19 year old ,about 3 months ago i started having problems with my penis.the foreskin is slightly swelled and looks semi white and ive got some build up under my foreskin,i think its smegma but not sure but i keep it whiped off and it comes right back and its very itchy and around the base is to.im a virgin so i dont think its a std and im diabetic and ive had urine anaylasis do and my liver counts are slightly high and kidney counts are very high and i think she said i was peeing out alot of sugar so maybe that has to do with it im not sure but ive been putting athletes foot cream and monistat on it and it calms the itching for a few but im afraid,if you need any more info pm me and if you can help me pm on here,thanks
This build-up you speak of, how does it look like precisely? Is it white powdery/chalk-like mass? Does it occasionally burn (especially when it's near the "hole")? If you pull back your foreskin, does it have traces beyond the "pullback borders"? Does it have a foul smell?
If the answer to these questions is ‘yes’, then we might have the same thing. I believe it has nothing to do with sugar (I'm not a diabetic btw). I believe it's a mass of dead oil-based cells (that's why water isn't as effective).
You should try using baby soap (remember, a PH neutral soap, otherwise you’ll hurt the tissue – and you wouldn’t want that, would you). It should clean it off, although they will come back in a few days. Idk why, but they just do.
Have you been neglecting personal hygiene lately? That could cause their appearance. Pretty much like a cold, it’ll occasionally come and go. It’s not something I would worry about.
Sorry for improper use of terms. I don't really know the exact medical phrases, so I had to use the 'vulgar' ones. Hope it doesn't mind though, and I believe it's pretty easy to understand what I'm saying.
I'll try to help you out even if none of the things above fit, but if all else fails, see a urologist (unless the doctors are total dbs where you live too).

ive did what ive always did take a shower every day or two cause i dont stink lol but ya its like a thick white paste and it does stink and when i pull my skin back to clean it,it hurts extremely bad but have to and my foreskin is kinda swelled and whitish and kinda cracked but i think thats from pulling it back while its swelled to clean it but should i keep using monistat or what to get rid of it,any tips to get rid of it im worried sick over it,its starting to affect me mentally and put it in any words i dont care i understanded it better anyway

Yes, your problems are related to sugar. High blood sugar and high urine sugar increase infection susceptibility especially fungal. It seems like all your problems are related to yeast (a fungal infection)--the leg rash and the foreskin/penis problem.
It also sounds like you MIGHT have a problem with tight foreskin (?), and yeast infection can actually cause the foreskin to grow tighter. This can be overcome with regular gentle stretching. Note: you might not even be able to start stretching until you overcome the yeast infection first (because the yeast will keep making the skin too tight). See the sticky thread "Stretching To Fix Tight Foreskin (Phimosis)" for more about that. Your tightness might also be an adhesion and/or frenulum breve. (That thread addresses all these common foreskin problems.) You will probably need to stretch it out enough to clean it better to get rid of the infection. You also need to stretch it out enough to retract fully and easily if you ever want to be any good at sex. (If it can't be fully retracted easily, the foreskin can tear and bleed from sex.)
The whiteness is smegma, an accumulation of dead skin cells and skin oil. And once we're old enough, there can be semen residue in it too, which makes it smell terrible if it's not cleaned. It's a sign you need to keep the area cleaner and dryer. Don't use a soap that leaves a residue on the sensitive skin (like Dove or Caress). The residue can knockout healthy skin bacteria which allows yeast take over. The yeast infection on the skin makes the skin overproduce new skin cells, which is probably the source of the smegma problem. You should wash your penis out every night before bed (and you don't want to masturbate after washing--that defeats the purpose). At least use baby soap. But an anti-fungal soap would be even more effective (as long as it's not too irritating).
There are a lot of dietary things you can do to minimize yeast problems. Avoid dairy (especially milk) and sugar at least until your yeast infections clear up. I like the natural artificial sweetener stevia. That might help you avoid sugar. You also might have a UTI, if the yeast is traveling up your meatus at all (which seems it could be likely with diabetic urine). Cranberry juice often clears up mild UTIs. There are also (sugar free) cranberry tablets that might be better for avoiding sugar.

So it should clear up,I need to keep it really clean and make sure no urine gets on it and if it does clean it off and do I need to use monistat or anything or what can I do or put on it to clear it up asap
Yeah, I guess I'd probably keep using the monistat until it clears up. And don't eat any sweets, sugar or dairy until it's better. That will help it clear up faster.
Wash it every night by gently rubbing all oil/residue off with the fingertips. And most importantly, try to keep the area as dry as possible. Yeast requires moisture to live.
You might find it helps clear up the yeast to keep the foreskin retracted so it drys out. Do this after washing before putting clothes on so that clothes are dry.
The most powerful anti-fungal soap I've ever used is Neutrogena T/Gel. It is very drying too. It might be a good soap to use on the infection (as long as it's not too irritating for your skin). (It's not too irritating for my foreskin, but everybody's different. Be careful to keep it out of the meatus.)
