I think that if there's ever any pain coming from that skin, you're stretching it too much. Even slight/tiny pain indicates some degree of skin damage. For growing stretchy new skin, you can't damage it, you have to go more slowly than that.
One thing about yeast, if the skin had been loose before and then one day it got too tight suddenly/unexpectedly, then it's very likely yeast-induced tightening. This is a well known concern for diabetic men (usually older), because their high-sugar urine promotes yeast infections. But this is not the only way that yeast might be involved.
It's normal for the skin to look a little bit frayed at the end (wrinkly). But might you be describing posthitis, inflamed foreskin? Is that possible? Posthitis is also a symptom of infections like yeast.
The tight skin encourages yeast infections, because it's not possible to wash or dry the inside when it's too tight. Itching is a really common symptom of yeast too. If yeast might be making the skin tighten, I would see a doctor for strong antifungal drugs and skin thinning cream to help stretching.
If you could have a yeast problem, it helps to lower urine sugar levels. Sweets, milk and cream make yeast problems worse. Cranberry helps. Washing well and keeping the area DRY helps. And I like pine tar soap (if it rinses cleanly). It's really good for yeast and other minor skin infections. It's very drying.
