Unknown Illness causing Fever followed by Mouth and Genital Ulcers

Hello Readers,

About a month ago I came down with a minor cold that progressed into a fever after a couple days. After a day or two in bed I began to develop lesions on the inside of my mouth and around both sides of my scrotum, mostly in the area that brushes against my inner thigh. The lesions in my mouth turned very painful, my mouth was swollen and I was unable to eat solid food for about a week. The genital lesions were painful and it was hard to walk for 5 or 6 days, but they healed up fairly quickly. The fever subsided in only 3 or 4 days. I have searched for illnesses that cause fever followed by both genital and oral lesions at the same time but have come up empty handed. Any help or suggestion is greatly appreciated.

A description of the lesions (size, how they progressed, itching?, etc) would be helpful, but this could have been a primary herpes infection. I would think about any possible exposure to the herpes virus. If it was herpes, it will likely recur at some point without the fever and much fewer lesions. If it does, it should be cultured or antibody levels checked. Antibody levels can be done now as part of the convalescent stage. There are other disorders that can cause fever, mouth and genital ulcers that fall into the rheumatological category, like reactive arthritis (Reiter's syndrome) and Behcet's (rare). If your illness included marked joint pains, one of rheum disorders is more likely.

Thanks for your response. The lesions were red, open sores around my scrotum. The sores in the genital region began with intense itching and progressed into open rash-like sores. Small, non raised bump sores formed around my inner thigh as well. Some of the sores were formed by cracks in the skin. The sores in my mouth progressed along both sides of the cheeks, along the roof of the mouth, tongue and along the gum line.

Did you see a Dr during the rash stage? I would talk to your recent sexual partner(s) and see if any have a history of herpes. I would also be sure to practice safe sex in case it was a primary herpes outbreak to avoid transmission to anyone else. In a recurrence, the sores will occur usually on one side of body only. Lesions will start with itching, possible tingling or a hypersensitive feeling to skin. and usually develop into red blisters before opening and turning into ulcer-like lesions. If you've never had anything like this before you could request to get HSV 1 & 2 antibody testing done. IGM (the acute phase) may be negative by now, but IGG should be positive at a high titer if it is herpes. Reiter's syndrome usually follows an STD like chlamydia or an infectious diarrhea about 3 weeks after the original infection.

I saw a Dr. post-genital rash stage. I found it odd that I had an outbreak of sores in my mouth and genital region at the same time. Not to mention the sores being so bad in my mouth that I could barely eat for a week. Is this common? I have had two sexual partners in the past year, one over eight months ago and one who I have been very close with over the past few months.

This site doesn't allow the posting of the link but I googled and found this reply to a question asking about simultaneous oral genital herpes-symptoms.

" One of the more common complications of HSV infections is the occurrence of erythema multiforme and/or Stevens Johnson syndrome with each herpes outbreak. The rash of EM can be subtle or in a site where it isn't readily visible, perhaps missed; S-J syndrome is EM plus genital or oral ucers. You might ask your doc whether this might explain your facial and/or genital symptoms, rather than HSV itself--i.e. true herpes at one site and EM/SJS at the other. Don't get me wrong; EM and SJS usually are not subtle, and this scenario is unlikely. But the whole situation is atypical, so perhaps worth discussing with your doc."
