| Psychiatrist passed away. Records?
Hello,
I went to a Psychiatrist about 5 or 6 years ago, because I was having trouble telling my parents about my sexuality etc etc. I went there about 3 times, mostly talked about why it made me nervous.
Here I am a few years later, I'm applying for a new job and on the medical history form I had to indicate that I have seen a mental health professional in the past (Federal job).
Unfortunately, the Psychiatrist passed away last year (old age).
HR for the agency is requesting that I be evaluated by a mental health professional (this is the easy part) but they also want copies of my records from the Psychiatrist I was seen by in 2002.
His office number is disconnected, he owned his own practice- is it possible to get these records? How? I have no real way to getting in touch with his office, since he WAS the office.
If anyone can give me any advice I would appreciate it. I plan on contacting HR and explaining the situation but thought it prudent to do a little research on my own first.
Thanks in advance....
