Under your toenail that's about to fall off will be a very soft, new toenail. When your old one falls off this new one will harden and take it's place. It may take a few weeks for it to heal completely but unless it looks infected, I wouldn't think a doctors visit is necessary. My finger nail fell off when I was 11 after jamming it in the car door and apart from the soggy new fingernail feeling gross, everything was fine.
If your worried about the appearance of your toenails then just wear some different shoes for a few days but otherwise, give it lots of air.
