Besides talking to your doctor talk to the artist you have picked to do the tattoo. Make sure you have picked a clean place that you feel comfortable with. Look at the artist book to see what his/her work looks like. If you can talk to other people who have been to the place to see how they feel. Talk to the artist about his tools,the devices he uses to clean those tools. If the artist doesnt want to take the time to talk to you about it or answer any of your questions turn around and find another place.
As for it being painful that is a matter only you can decide. I have 8 tats and I can honsetly say that only one of those seemed painful to me and that is the one on my hip. I even have one on my left foot and that one didnt hurt to me at all. The other places I have tats are both my shoulder blades,left breast,left arm and right calf.
