Assumign that it isnt just a tight muscle that happens to be in that area...where did he get this tattoo done at? A respectable shop, or someplace that is 'underground'? There are plenty or tattoo 'artists' that aren't even licensed, and therefore aren't taking all the precautionary measures that they should be to ensure that their customers are safe. It's quite possible that whoever did this tattoo was inexpierenced and used too long of a needle for the procedure.
If this was the case, it's very possible that he has some nerve damage or damage of some sort. If it were me, I would go back to the shop where it was done and ask the artist about it. Ask to see their health inspection forms if they arent posted, and make sure everything is ok with that. Maybe they will even look at it for him and see if they can figure out if there is something going on with the tattoo that the untrained eye wouldn't notice. If that doesn't help any, then I would suggest off to the doc. Then if it is some kind of damage, then it can get repaired before it gets any worse
