Has he been playing outside where he could have come in contact with poison ivy, poison oak or poison sumac? Those most likely attack the hands (from touching it) and feet (from walking in it). While those rashes most likely itch, they can become painful if a person is super sensitive or if scratching it.
Try some calamine lotion on it. Do not let him scratch the rash. If it starts to spread significantly, get him in for a doctor visit.
