antibodies from blood transfusion

My mother is in the hospital and required a blood transfusion, which she got, but now she has antibodies in her blood and they are having a hard time finding a donor with that same antibody to give her another transfusion. Is that hospital error? Meaning that they did not give her a perfect match when they transfused her and now her blood has created these antibodies, or am I missing something here?
