Torn Volar Plate

I was ramping my bike 3 years ago and fell. Since then I have had pain in the PIP joint in my right (dominant) hand. The x-rays showed nothing during my first visit which was 3 months after the injury. I was told to squeeze a tennis ball. I was having pain, swelling and was unable to straighten my finger. After 4 months of that I was told I probably broke it and needed to break up the scar tissue. I soaked it in hot water and stretched multiple times a day. After about a month my finger was straighter but not perfect and still swollen and painful. I was told to splint for the next 4 weeks. After that I saw an orthopedic hand surgeon. I found out I tore the volar plate and for some reason it didn't heal. I had 2 rounds of physical therapy and nothing got better. I had a 4th opinion and nothing was done. I have not had treatment in over a year and a half. I am still having extreme pain and swelling. What can I do to help myself? Why hasn't it healed? Should I push for surgery? Now I know surgery is rarely advised and may lead to worse ROM, but I am willing to live with that if the pain would go away.
