Red face Bunionectomy screw removal?

I had a bunionectomy-fusion a few years ago on my big toe, and had a screw inserted in the side of my foot.
Last week, I noticed that the bump where the screw is has begun moving away from the scar, towards the bottom of my foot, and it's been uncomfortable...

Could the screw be coming out of place? Should I go see my orthopedic surgeon, get it checked/x-rayed and possibly removed?

I can see it sticking through but when I had the surgery I didn't notice any bumps or anything, and it sometimes throbs if I walk for longer than a couple minutes.

My surgeon has told me in the past that if the screw was bothersome and needed to come out, it'd be done with local anestetic.
I am, however, petrified of needles... so would I be able to be put under for hardware removal?
