Cost of Mid-Foot Fusion

I have a 14 1/2 year old injury to my left foot. It was repaired with 3 pins & 2 screws. Since that time all but one screw has made it's way out of my foot. I am now suffering from severe arthritis which has deformed my foot, I have no clean joints and it is getting increasingly more painful to walk.

I just saw a specialist who indicated that I need mid-foot fusion. They will need to clean up the joints, remove the last remaining screw and put in plates.

I just accepted a new job 2 days before I saw the Dr. and now have to switch insurance. I won't be able to keep my Dr. (who is awesome by the way). There are several different plans that I have to chose from with my new employer and the cost of this surgery will play a huge role in determining which insurance I chose.

Does anyone have a ballpark on the costs for this type of outpatient surgery?
