i had this same problem with my PM doc on the first day.they believe that you should only need long acting pain meds and if you need BT meds that they need to raise the LA dose.Well, the type of pain that I have causes these sudden horrid jumps in my pain levels so the raising the La dose theory just dosen't work very well for me.
i had a discussion with my NP the (nurse practioner rxes the meds at my clinic,the pM doc does the procedures)about why I needed the BT meds and told her that you cannot treat everyones pain in the same way as no two people will feel pain in the same way and there are so many different types of pain.You need to treat pain individually.well, after this discussion, I did start to get two oxy IRs per day.It isn't ideal, but i am still working on her.My point is is that you really need to have a very in depth chat with your doc to explain just why this method of pain control is not working for you,and what you feel would be the ideal way for you.Hopefully, you will fall somewhere in between.Since it sounds like you are going to have to stay with this Dr, you two need to know where each other is at here you know?Good luck,i hope this helps some.Marcia
