I too am so sorry you are having to deal and live with chronic pain. I'm new to the pm thing, but I can offer some advice that's helped me:
First and foremost, have all of your ducks in a row as far as when your pain started, what procedures you've been through, medications you've taken in the past for this pain and the results you got from those medications, know dates of procedures and medications taken. Anything having to do with this condition. Lots of this information will be in your records that are forwarded by you referring doctor, but, it's good to know this information to be able to correctly fill out the multiple pages required by your pm doctor. The more thorough you are, the better equipped your new pm doctor will be to help you.
Be open and honest with your doctor about things that have and haven't worked in the past, and try and write any questions you have down before the appointment and take the list with you. It might help you to keep from forgetting anything.
