Question Is this an allergic reaction???

I have been on morphine since july 06, first dose was 30 mg twice daily with norco for b/t. Next dose was 60 mg twice daily with 15 mg MSIR for b/t I have been on this dosing for 6 weeks. I have been feeling just aweful, moody, and just a rather ugh feeling. I also have been feeling dizzy and have occasional double vision. I also have been extremly tired. I was thinking it may be the increase at first but it hasn't gotten any better, I don't like feeling like this it is hard to concentrate too. Is this a normal reaction to morphine? Is this an allergic reaction? The pain level went from a 9 to a 6 but I am still having quite a bit of pain. I have an appointment tomorrow and I think I am gonna ask for a med change but will i have this reaction with other meds too?aren't they all morphine based anyway?

Hi Dubm, It's definitely not an allergic reation. It's simply side effects of taking more morphien than your used too. It would have made more sense to go to 45 mgs of lobg acting morphine twice a day rather than doubling the dose, But you can't break LA meds in half and not destroy the release mechanism. I would defintely tell the doc it's taking a long time to adjust to this and if 45 mgs would provide ebough relief without hthe side effects you either stay ther or stay there untill side effects deminsih and then step up tp 60.

All opiates are not morphine based. Morphine,heroin and codeine are drived from the poppy or a chemical n the poppy. They basically duplicated the chemical structure synthriclly and created a fgorup of similar drugs called keto synthetics, Hydrocodne, oxycodone, hydromorphone "dilaudid" and Oxy morphone fall into the family. There is also another class or model for synthetics that Fentanyl and demerol fall into. Opiates or opoids, whether they were originaly derived from opium are all synthetically created now. Opiate synthesis where opium isn't even needed to produce the natural opioids "morphine & Codeine" has been around since 1979.

But an alergy or side effect to one class of opiates doesn't mean another class will have the same characteristics. Most people aren't allergic to every different opiate class . Just specific classes. If your alergic to morphine, your would likely be alergic to codeine, but you wouldn't neccesarrily be alergic to all classes.

It was a big increase, doubling at that level is safe as far as safety goes, but it's really the side efects that limit the dose someone can take. If the side effects are more dsisabling or imparing or not worth the trade for relief, you have to cut back and find some middle ground where side effects are tolerable and you function better due to meds.

Morphine is the gold standard to which other opiates are compared in strength, but hardly the strongest of all the opiates. They basically lok at the chemical structure of morphine and tweak it a little and create a stronger version. Consider opiates like a puzle pieces that fit into opiate receptors. The better the fit, the more effect it will have. They have synthetically created drugs that fit better than morphine, but you could still say all opiates are morphine like.


Changing meds at this point would be a shame, because you only have so many to choose from. It really comes down to what you can tolerate and how long you tolerate feeling bad before the side effects can deminish with time. A couple more weeks may do the trick and most of what you described may demish. If you give up now you basicaly start completely over trying to find the right med, the right dosing interval and you have wasted this time and have to wonder, did you give it a fair shot before throwing in the towel on morphine? 6-12 weeks to find the right dose and accomadate to side effects doesn't seem that long compared to years of chronic pain.
Good luck and no hyves, no dificulty breathing=no alergy, just side efects.

Thank you for the advice. I appreciate it. You are right a step from 30 mg to 60 was a lot and it was horrible. It was a total toss up between relief and side effects. I had the worst constipation imaginable. I went to the doc yesterday and told him my symptoms he brought me back down to 30 mgs twice daily but changed the b/t med from MSIR to Percocet 5/325. My only fear was that i would be poorly under medicated now. I guess I will roll with it this month unless it gets too bad and then i will call him. I can handle pain and being under medicated. During my last visit i had an esi injection w/ sedation, he was supposed to refill my MS COntin and MSIR but he forgot to refill the MSIR. SO last month I had to step down from 6 per day to 4 per day and the last week I had 2 per day. Last week sucked by the way but I made it. He was impressed that i just handled it myself instead of calling him for more. Anyway, thats the latest This sould make my side effects diminish. I will let you know how the percs are doing. Again, thank you.
