You are truely a great friend, living with pain is not easy. I had severe pain on the left side for more than 10 years. I had every test imaginable and because it went on for so long the symptoms differed but the pain in the left side was always there. I agree with sammyo1 don't give up and don't trust what the docs tell you always question it, they don't know everything about human physiology. When they can't find a reason the they give it a fancy name or say its ME/CFS, which covers everything. I will not go into my whole history but I can tell you that for the first few years I hid the pain. I did however end up in accident & emrgency many times because the pain was so severe. I always felt my doctor and specialist didn't believe me because blood test scans etc showed nothing.
Symptoms I had over the years were similar to heart attack, chest problems, kidney problems bowel problems and many more. There were pains in the groin head legs chest stomach.
It was only when I went private and had them remove my appendix which was not the problem did we disover the cause of my pain. I had what was called a Richters Hernia with abcess. I had a small hole in my pelvic bone which my intetine had grown into (imagine blowing up a balloon against a wall with a hole in it, the balloon fills the hole) and behind it grew an abcess. When they took my appendix out using key hole surgery they dislodged it and the abcess burst. They didn't know at the time so sent me home the next day. I almost perished the next day because of the poisoning. They rushed me back split me open and saw what happened. I was lucky someone saw me deteriate or I wouldn't be hear.
Cutting it short a Richters Hernia affect women usually and is very rare at that, for it to affect a man is virtually unheard of. The sad thing is doctors still don't believe it happens and there are no test for it. You cannot see it on scans or xrays, the only way is by Laparoscopy and doctors don't order these unless there is a real need to do so.
Your friend may not have exactly what I had but it is worth checking out Hernias and cysts. Don't let him give up and accept the pain, for me behind the pain there is aways cause. I did give up at one time but the pain was so bad I could not ignor it, pain management didn't help even the spinal lumbar something another where they block certain nerves.
The symptoms your friend had I have had, so no one suspected a hernia.
