I would agree. The morphine pump should be one of the last options. Morphine from a pump is like morphine IV in a hospital.
The reason oral morphine doesn't work that well in some people is because it has low bioavailability. Think of bioavailability as "efficiency" and like miles per gallon (MPG) in a car. Well, oral morphine has poor MPG. You have to take a lot more of it in order to get the same effects as other meds.
As Brian said, there are many other meds out there to try before going to a surgical implanted pump. If your aunt isn't all that active, then the patch may be a good bet...It works well.
