Question Fentanyl patch adhesive question

Hi! I just started pain management about two weeks ago (I see my Doctor on Tuesday Sept. 1st) for a follow-up visit. Prior to this my pain management had been handled by my orthopedic surgeon. But, it got to the point where my quality of life deteriorating that both my surgeon and myself felt that I needed a long acting med (besides the short acting Norco) I have been taking.

My Specialist is wonderful, she spent a lot of time with me and talked to me about various options - injections, etc. and long acting medications. She started me on the Fentanyl pain patch at the lowest available dose - 12mcg. per hour. However, I noticed very quickly a major difference in how I was feeling - at first I was hoping it wasn't the placebo affect, but I have found that I have slowly become more and more active again and feeling like my "old self" again and actually felt like I have been "coming back to life". I am also taking Norco for breakthru pain and am down from 8 per day to 4-5 per day. Anyway, the patch I am is the Mylan brand and up until today it has been great - sticking to my skin well with no issues.

Well, today I changed my patch and it seemed to stick like it was supposed to and I also did what I was instructed by Doctor - apply a bit of body heat to it with my hand to ensure the adhesive stays in place.

Well, I took a walk with my husband earlier and to perspire a bit (it's been very hot in my area). I also cleaned my son's room as well. I didn't think about it until I touched it this evening and noticed it was coming off. There doesn't appear to be any adhesive left on it! I did take a piece of medical tape and taped it in place. But, I can't help bu wonder how effective it is, if it isn't even sticking on its own?

I'm not sure what to do. I don't want to start experiencing too much pain and I don't want to have to go back up on my norco (I have been so happy with the results and taking less pills). My Doctor only gave me enough patches for two weeks - 5 patches that I change every 72 hours. So, I only have enough until my follow-up appointment. I don't want to bother my Doctor's office either.

I could be overreacting, I am not experiencing increased pain at this point. I'm just wondering if the patch is even effective. So, at this point it is taped and my question is it still releasing medication?

I apologize for the long post any suggestions are appreciated.

Amethystma has provided you with a very good thread. I won't repeat what's there, but I will say that as long as you get the patch back in contact with your skin, you should be ok. It must be in good contact with the skin to work properly.

If you know in advance that you will be doing active type things, then you might want to tape it down with the tape covers right off the bat. Some of the tape covers are less sensitive to sweating & etc than the adhesive on the patch.

Skin sweating is probably the biggest drawback IMHO to the patch.....Especially during the warmer, humid months. You'll find it to be a lot less on an issue once the weather turns.

A couple of tips for you...Try to apply the patch to an area where you have less sweat glands. For example, the area on your outer arm (where people get a tattoo), or on your flanks. Both areas are highly recommended in the manuf. directions. Stay away from the stomach area, and your back...lots of sweat glands.

Also, clean the area where you put the patch very well. A good clean area makes for a much better seal. Using your hand to create heat to seal the patch is a good idea and recommended by the manufacturer. I know it can be tough to do, but hold your hand over the patch for 1-2 minutes.

I'm guessing you're on the Mylan patch. You could try the Sandoz version.

Thank you so much! I read through the thread and that helped a lot. Also, I have felt fine, so apparently my worrying has been for nothing. My pain has not increased at all. Thanks again, the thread was very informative!

Thank you. I will definitely keep that in mind especially now that I'm becoming more and more active again. I appreciate the input.

Yes, I am using the Mylan patch. The funny thing is they have been sticking perfectly fine (actually better than I expected until this most recent patch). I will definitely ask my pharmacy about other brands. Thanks so much!

Hi there,

I've been using the Duragesic patch for about 2 years now. After wasting many of them (because they fell off) and trying almost every tape on the market...my husband found this tape by Nexcare (3M). The exact name is First Aid Absolute Waterproof wide tape by Nexcare. It's 1 1/2 inches wide, slightly cushioned, flexible and stretchy so it has some give to it. It really feels like a 2nd skin. It's also hypoallergenic and and latex free. I get it at Walgreens for about $7.00. It's beige in color and it comes in a roll..so you can cut it to whatever size you need. I used to cover the whole patch but now I've just been covering the edges. I have never had one fall off...not in the shower and not from sweating.

Your other choices would be Duragesic brand, Watson, and Sandoz. I have also listed them in order of potency with Duragesic being the highest. Mylan is typically the least potent, if you switch, keep that in mind.

Also, Duragesic (Janssen) makes Sandoz (generic), but they use better glue on the branded product, so it sticks much better. I've tried both and Duragesic brand is far superior (to all others).
