Hello all, hope you all are feeling good. Been taking 2 30mg roxicodone every 4-6 hrs, max 5 total pills a day for about a year. They have become not helpful, so pain dr switched me to 1, 4mg dilaudid every 4 hrs, max 5 pills a day. Based on a pharmceutical conversion of 150mg of oxycodone daily converts to 56.25mg hydromorphone daily. When i mentioned the conversion she said , yes thats correct but dilaudid is faster acting and is stronger so lets start off with this and go from there. Its been a week now of taking 1 a day every 4 hrs and iam not getting the relief like i was hoping too.I understand the conservative approach, but from your personal experience what strength do you think i should be taking?? I always appreciate everybodys input and advice.

Sorry you’re in pain. I was on the Oxy IR for a couple of months having to take 20mg every 4 hours and I was still experiencing pain, plus the side effect of constipation wasn’t exactly pleasant. I asked my Doctor if he could switch me to Dilaudid because I had been on it in the past and it seemed to help me much more than what the Oxy IR did. I am prescribed 4mg every 4 hours as well. From a discussion I had with my PM Doctor, it takes your body a little while to get used to a new type of pain medication. So hopefully by hanging in there, you will begin to notice a difference in your pain level. When I switched over it took about a week and a half – two weeks before I got full benefits from Dilaudid.

Dilaudid has been very helpful for me and works with my body better. I hope some of this helps and your pain begins to decrease. Best of luck to you.
