Treatment with Benzodiazepines, antidepressants and antipsychotics

I did not know this but the manufacturers of the 3 major benzo's used to treat panic disorder are recommending that in severe cases, individuals can go up to a maximum daily dosage of 10mg a day! The 3 major benzo's, i am referring to are xanax, klonopin and ativan. The most ive seen pdocs prescribe are 4mg per day, (if they are willing to prescribe them at all) but usually the maximum daily dose is less than this. Often we see patients not respond to these low dosages and he/she and the doctor will give up. Also, some patients may be rapid metabolizers of the benzo and need an increase in dosage. Imo, i dont think doctors pay much consideration to this.

Now in most cases once the therepeutic dosing range is established the patient will not develop tolerance to the medication. And as far as weaning patients off of the benzo, what makes a difference if its 4mg or 10mg? Obviously the latter will just take longer. And if the 10mg is working, why would you ever need to be weaned off anyway?

But what do doctors do instead? Precribe antidepressants which clearly have more side effects or antipsychotics which have even more. If the antidepressant does not work, another one might be added on or an antipsychotic with even more side effects. As far as withdrawal goes, withdrawal from antidepressants can be just as uncomfortable than the benzos.

Imo and im sure many others would agree, pdocs are being enormously mislead by fortune 500 companies such a pfizer, lilly, forest, smith kline and others. Benzodiazapines should be the main stay of treating panic disorder and if everyone here was on the proper dosage, there would be fewer and fewer posts of complaints of symtoms and more and more people away from their computers and out enjoying their lives.

the benxzo dose prescribed is usually too small and some do take high doses daily with no harmful effects, however caution should be used with higher doses, IMO I know of severa psychs who specialise in panic attacks, they all prefer benzos and wont use any antipsychotics

Re: Treatment with Benzodiazepines, antidepressants and antipsychotics

Ya know I've been on Xanax for about 5 years now at .5 a day. Thats not much. I'm ready to go off, but its been a killer. I taper very very slowly and feel every bit of it. I'm glad the Dr.'s are at least trying to be conservative about prescribing these meds.

Right now the w/d and mild tolerance I have is tempting me to raise my dose to 1 mg a day. God knows I can use the break from the headaches and anxiety and panic, but the fear of eventually having to taper and go off is always there. If I do miss a dose, I feel it immediately and it is real wicked. Almost like I loose myself. My brain fogs ups, muscles ache, nervous anxiety, etc etc. My body has become dependent on the drug.

I'm glad I took the drug in the beginning, as it helped me so much to cope and help the panic attacks. But now I feel I rely on it to feel "normal".

Its a hard call to make and I would just warn everyone before they start a benzo to know that that can be very difficult to get off of. Especially those who take them for a long period of time.

Re: Treatment with Benzodiazepines, antidepressants and antipsychotics

housemum,

I also took 1/2 Mg of Xanax everyday...I took it for 7 years.
I have a question for you....did you take the whole 1/2 MG at once or did you take it 1/4 Mg at a time?
I'm asking because I took mine 1/4 MG at a time...usually about 8 hours apart and when I weaned off of it I didn't make a cut in the amount I took at first, I just took it farther apart...ie...instead of taking it 8 hours apart my first cut was to take it 9 hours apart, then 10 and so on until I was taking it at 12 hours apart.
Then when I got there I cut 1/4 MG in half and took 1/8 MG every 8 hours, then every 9 and so on until I got to 12 hours apart.
Then I took just 1/4MG a day then every other day then every third day and after I was to the point where I was going to be taking it every fourth day I just stopped and I had no WD symptoms at all.
Like I said if you were taking it all at once this wouldn't be a way you could do it and even if you were taking it the way I was the way I weaned still might not work for you, but I just thought I'd throw it out there to see if maybe it might be something you could try.
I'm by no means an expert, but just thought it might be of some help.

Forgot to mention that I usually made these cuts every two weeks...but there was once or twice I stayed on a dose a little longer because my Doctor told me that my body would 'tell' me when it was ready for another cut...and if I started to feel bad to just stay at the dose I was for a little while longer.
