13 weeks and I'm a wreck-- ultra sound showing marker for Downs

I'm 37 years old and 13 weeks pregnant. My OB suggetsed I see a specialist for a more detailed ultra sound and blood work to rule out any chance of congenital disorders such as downs, trisomy 18 and spina bifida. I did this with my first pregnancy at age 35 and all the tests came back negative. I went to the specialist on Wednesday and the ultra sound revealed 3.21 mm of extra spinal fluid on the back of baby's neck indicating a problem. I am beside myself and have been crying for days. They drew my blood that day for testing and they will draw more blood at 16 weeks. If the first round of blood work comes back positive they will procede with CVS testing where they take a sample of the placenta. CVS has a high rate of miscarriage and I'm just a wreck from all of this. Has anyone had an ultra sound indicating markers for Downs and how did your blood work come back? Could this be a mistake? Should I get a second opinion? I'm so upset I can't think straight. The blood work will take about a week. I'm trying to stay positive and any input would be helpful.

I'm sorry to hear you are going through this... but just remember that these are markers and not a diagnosis. I have read too many times of people's stories where they have a high marker, even higher than yours and then their babies turn out fine. I'm sure there are others where their babies do have problems, but what I'm trying to say is that this is the first step marker, not a diagnosis.
I would be upset too, but try to relax until they can do more tests and can get a better picture.
I think you have two things you can decide here...
With the first round of bloodwork- once that comes back, you'll have a better sense if you want to try for the CVS. Or if you don't want it, by the time they do the 16 week blood work (quad panel screening), that combined with the first u/s and bloodwork will bring your results to about 85-90% (if I remember correctly) as far as whether there is a problem.
I think you should think about what you would do with a child with special needs... would you still keep the baby? abort it? These are your personal decisions that only you can make. If you would keep the child no matter what, then maybe you skip the CVS (so you don't have the miscarriage risk) and wait for the 16 week bloodtest to help confirm things. If you think you'd abort the baby, then maybe you take the risk of the CVS to have absolute confirmation early enough.
Don't make any rash decisions... be honest with yourself and ask a ton of questions with your doctors before chosing to do or not do a particular test.

Liloulou makes some excellent points. I really liked that she said "be honest with yourself." If you are a person that truly thinks they would be ok with a child with special needs, then skip the CVS - it is risky.

Also, liloulou is SO RIGHT when she says that the test is only a marker - NOT a diagnosis. Hopefully that puts your mind at ease a bit. I have also seen/heard of women who have had high markers and had "normal" babies.

Don't let this test discourage you about the life inside you. Positive thoughts your way.

I skipped all of these tests. In the end, it didn't matter much to me. Whatever is there, will be there when my baby is born and there will be plenty of time to deal with it then. I really just wanted to enjoy my pregnancy without the worry and stress of tests, results, complications, etc. The best advice I can give you, is to not borrow trouble. They are just markers, not diagnosis. It's hard to just put it aside, but I hope you can find a middle ground with it all that says, enough is enough, this is my child, and I love him/her no matter what..........and then leave it there. If not, then you can allways go for the cvs. I wish you the best of luck with whatever your decision is!

Thank you all...I have been pretty nervous from the begining with this pregnancy. I'm not sure why. I have been doing a lot of research on the internet and I feel a bit better. I have heard of many woman having higher limts on their ultra sounds than mine, and their babies were born just perfect! I'm trying to stay positive but also have to prepare myself for the "what ifs." It's been a roller coaster ride to say the least. I've decided to get a second opinion at a different perinatologist office. I'm hoping the blood work will come back okay and we won't have to worry about this anymore! I can tell you one thing...I won't go back for any other diagnostic testing which they want me to do at week 16.

This same exact thing happened when I was pregnant with my son. They sent me in because they "thought" he had fluid around his neck. They did the blood work and I was a complete wreck as well. Turns out the kid just had a big head and thick neck. No downs here. He is perfectly healthy and turned two on the 29th of June. I just found out I am expecting my second!!!!!! Keep your chin up and your spirits light. Have faith and you will make it through anything.

Aphrodite--Did you end up getting an amnio or CVS? I called the doc today and they can't release any information because I haven't had the second part of the test yet. I have to have another round of blood work done in 3 weeks and apparently they will know the results then. My doc said last week that I would find out in 7 days whether or not he wants me to come in for a CVS--the docs office gave me the opposite info...I'm still waiting to hear...ugh!! It's agonizing as you know! I'm trying to stay as positive as possible. Thanks for your words of encouragement.

when i was 21 i had the blood tests which came back showing i had a increased risk of having a downs baby like you i was distraught and so upset they offered me a amnio straight away where they take a sample of amniotic fliud from around the baby and test the genes for downs i know with this procedure there is a slight risk if misscariage, it was a done at 16 weeks and i had to wait 3 weeks it was agony, the cvs you are on a bout carries a higher risk of misscariage than the amniocentesis so you could look into that, after the 3 week wait i was told everything was fine so try not to worry to much good luck, im 8 weeks pregnant now an 37 like yourself and im terrified when the times comes for all these tests so your not on your own chin up let me know how you get on.

No I didn't end up having to get either one of those. I got the blood work done and my risk was so low and there were no further markers they ruled it out. You will be alright. Listen, I thought about you last night as I was battling nausea and I wanted to tell you that having a child with Downs in not the end of the world. I worked with kids and adults with special need all the way through highschool and college. You would be suprised how very intelligent they really are. Difficult, but intelligent. They don't see things the way we do and sometimes that can be a very beautiful and enlightening thing. One of the kids I worked with is now starring on The Secret Life of an American Teenager. I didn't know until I saw an episode recently. They can do anything they put their minds to. It just takes them longer to understand the social aspect. Keep smiling you will be blessed.

Thanks so much for your words of encouragement! It's funny because before I opted to have all this testing I was having a discussion with my husband about whether or not we would have the testing done. Not in the least did we ever imagine we may have a sick child so we thought what's the point of the testing. I spoke to my ob and she said God forbid the baby has heart problems etc...you want to deliver at a specialty hospital in case the baby needs medical attention. That made perfect sense to me so we decided to move forward with it. I really wish we didn't because I'm not able to enjoy my pregnancy like I did with my first. I worry all the time. I told my husband last night that I just need to move on as our little bundle is fine and start enjoying the pregnancy! He agreed. I don't think I would have this testing if I became pregnant again. I would take my chances. Will you have the same testing?

I really appreciate your response, truly. I have thought and thought about what it would be like to have a special needs child. I certainly don't think it's the end of the world...I would take on the challenge. It would be difficult and a lifetime commitment as with any child, however different. My husband on the other hand is in total disagreement with me. He feels it's something he can't handle and feels our marriage couldn't handle it either. He feels by having a DS child would cause us to split -up. I'm in a terrible spot because I don't want to terminate. I don't beleive in it and I can't imagine what it would be like having this child growing inside of me for almost 20 weeks and then instantaneously have it be taken away from me. It's an agonizing thought. I'm just praying I don't have to make this decision and everything will be okay. My concen is whether or not I can afford to take care of a special needs child. I don't even know where to begin!!

You know, none of our children are "perfect". I have all outwardly healthy children. However, each one of them has their own unique problems. My oldest isn't ADD.....but it is definately a challenge to keep him focused if he's not interested. And he lives in "ME MYSELF AND I " land. Very hard to get through to him. My middle child was speech delayed. We later found out he was tongue tied. An easy surgery, but lots of speech therapy afterward. My youngest has a temper like no other. I've never had a fit thrower until now. I'm completely baffeled at how to deal with it. And these are the larger issues. Each and every day presents its own set of challenges.

I think your husband is scared....and therefor saying a whole lot of things in which he knows nothing about. I think I'd be standing much taller right about now and tell him to make his choice now. Before you even find out whether or not your baby is d.s. But I certainly wouldn't let him sit there and tell you that this baby could rip the two of you apart. That's sooooooooooo wrong. He'd be chosing to leave...plain and simple. And if he did that....however painful that thought it....you make sure you can take him for everything you can get. Now you can affoard your baby.

I'm sorry to be so blunt about it. But I'm pregnant and the father is gone because I wouldn't "get rid of it". I have no tolerance for a man who can create a child, but doesn't have the you know what's to raise it. I decided good and quick I'd be fine on my own, and so will you if it comes down to it. I really don't know what im gonna do, my sister who was 37 at the time had a one in three chance of having a downs baby from them (sorry) stupid blood test, so she had that special ultrasound that checks the baby's neck and would you believe it her results then came back as one in 15.000 so it just shows you them blood test ain't so accurate, i know a few women who have had the same problem. I chose not to have the blood test with my second daughter and she was fine to. With my first pregnancy when they said i had a one in 200 chance of having downs baby , my daughter was perfect and now she is 15 years old and extremly clever. I must admit im so scared and part of me wants the test and the other says don't do it. So what have you had so far in the way of tests ?

You probably had your test too early. There is a limited time frame to do the triple screen. Between 17-21.6 weeks I think is when the results are most accurate. Otherwise, you could get a false positive. Just keep your head up. I opted not to have any tests like that done. There was no way I wanted to know! I wanted to just get the hand I was dealt, and if there was something, so be it!

At 12 weeks I went to the perinatologist for an ultra sound of the baby's heart, neck-limbs etc...then they took blood. This is when they found the extra fluid on the back of baby's neck. I go back at 16 weeks and they will do another ultra sound and more blood work. all the tests compiled will give me my final result and odds of having a healthy baby. If I had to do it all over again I would not have this testing done. All it does is make me nervous...difficult to enjoy my pregnancy. I beleive too that you "deal" with the hand you're dealt, but of course it's a personal decision. My sil feels the same way. She was pregnant with twins in her late 30's...her first U/S showed a marker for DS and she decided she wasn't going to do any more testing. she just kept her head high and hoped for the best...she had 2beautiful babies born 4 months ago and healthy! I know how you feel and I can only tell you what my experience is...hopefully it helps make your decision...Good Luck!
