Mysterious Wrinkled then Swollen Fingertips pad/pulp regions

I have Been to 5 specialists. Did all bloodwork. A and A antibodies, serum levels. Nerve testing, UCSD Dermatology, Tried nerve blocks, etc.
THEY ARE ALL STUMPTED.
