sierra brulee

For some reason, every time I come I cannot log in with my name and password. I have to request a password change each time, more than once usually, and go through the steps to change it to one I remember. Since this is happening on different computers, and every time, clearing the cookies or the history is not working. I cannot figure out what it is that's going wrong. Help. Thank you.
