Yes, I think so. I once knew someone who was almost just as deluded and narcissistic. He may be a little bi polar. Some of the symptoms are lack of impulse control and the utter lack of ability to see how his actions affect others, he can only see how things feel to and affect him.
Obviously someone who is being unfaithful to his wife, breaking his marriage vows and lying every day to his wife's face is less than a good. loving husband. He's not thinking right.
I'm not sure what you can do about this situation. Since you are much closer friends with him than with his wife, it's not really your place to tell her, but I can understand your frustration and confusion. Maybe you can just be honest with him and tell him you think his affair is a horrible, dishonest, dishonorable, crappy thing for him to do and you'd appreciate it if he wouldn't talk about it or show any signs of it to you.
