Yep, that's really the best way. Sit him down when things are quiet, not in the middle of a ballgame or anything, and tell him you're feeling he's becoming distant, he's pulling away, or whatever it is that makes you think he wants to break up, and ask him what's going on, calm and without accusing or throwing a fit. And don't be afraid of the answer. If it's time to move on, it's time to move on. Avoiding it won't change that fact, it only prolongs the agony, like pulling a band-aid off slowly.
