Some guys simply have trouble that way. Sometimes it may be a change in her rythem that causes a guy to lose it, or there is less sensation from just her lips as opposed to her entire vagina, maybe she isn't going deep enough or using too much tongue or not enough tongue, or maybe you can't thrust, or it could be other things. Sometimes it is nerves about wondering if this time you can climax or not.
I have no advice on how to make it easier to orgasm that way. Maybe show her how to stroke your penis while going down? Good luck!
