I think you're asking two questions. I can't answer personally on the first because it's hard to mistake. However, at the height of arousal, it may be possible for some who are less experienced not to realize they did until it was too late. Perhaps someone can answer that better.
On the second question, it's very possible to be aroused and not ejaculate, for a variety of reasons. Alchohol and anxiety are two contributing factors either separate or in pair, that can prevent the natural from occurring.
