Question Pain when ejaculating

I have been researching this for my boyfriend and hope someone has any answers.
When he was about 13, he had an operation to correct twisted veins in his testicles. He first had intercourse at about the age of 17. (He's now 36)He had pain mainly in the head of his penis I believe, and has every time he has intercourse. However, he has no pain when masturbating. Partners, positions doesn't help, which confuses me, and him.
I would love to hear what input anyone has.
