It is all about the geography of the area - the colon feeds stools down which get delivered by the rectum when you have a movement. If you are a bit constipated then the rectum gets full as well. So when you try to clear the bowels you may only clear the rectum.

As with most things to do with the rectum, you have to be careful, and if it hurts that is a pretty good indicator that you may be going a bit far, or that you should go a bit easy. To help you make it easier try giving yourself an enema, which will make more room. It will benefit you not to be constipated, and then there is less chance of any pain or damage.
