If so, and in combination with your previous history of possible nerve damage, this sounds like you ***may*** have Pudendal Nerve Entrapment (PNE). Cyclists get this due to too much pressure on that nerve from their bike seat. It can cause pain and possibly impotence (yes, I know that is not your problem but left untreated it may).
It is often misdiagnosed as orchialgia. That basically translates into "pain in the nuts."
It is treated by physical therapy. If not helpful, doctors will then try steroid injections. The last resort is nerve decompression surgery.
Keep in mind that orchialgia is often over diagnosed for anything that a doctor does not otherwise know a diagnosis. I was diagnosed with that from the doc that I told you about earlier that wanted to do "exploratory surgery" without knowing why so I ran like hell. Damn good I did.
Be extremely cautious of any surgery involving your testicles as it will affect the nerves around each spermatic cord that goes up each side of your abdomen around your bladder. So, you will feel pain all the way up there. Even guys who have had a testicle removed still have that pain and it still feels like they are kicked in the nuts. It may be "phantom pain" but it is totally real.
Approach the doctor that you trust with the possibility of PNE and see what he says or if he can refer you to someone with experience treating PNE. Perhaps at a major college or university that may see more cases of this from their athletes.

If this is not the case and you indeed just have tight nuts causing the problem, you may wish to find a way to keep them warm all the time. Unfortunately, that means wearing warmer underwear. Just don't wear underwear that is so tight that it pulls the boys up too close. I had this problem anytime I wore tight pants. I don't have a pair in the house now, I wear boxers a lot and I don't wear anything at home.
