Not sure if I'm using the condom properlyy...help!!!

My gf and I have been having sex for some time. She usually helps me put on the condom. After its been completely unrolled, we pinch the tip to remove excess air.
Sometimes I would fall out of her and place it back in again two or three times each time we do it. Is that ok? Will that wear the condom? Also, at times when I slip out, I notice that the condom has moved to the inside of my foreskin (I'm uncut). So i make the adjustment and continue. Sometimes its uncomfortable so I adjust it by moving the front portion of the condom around. Is that ok?
Lastly, if used properly and I removed all air, its not going to break right? It's not some cheap condom or expired ones...
How much exactly does it take to break a condom? We left an unused one on the floor and I picked it and noticed tiny holes. I filled with water to confirm it with what I'm seeing. But I guess that could be due to the dirty floor and something rolling over it.
I'm just a bit nervous that I may not be using it properly.

You can do an internet search on this topic for such instructions.
I am concerned about condoms slipping off of you. You should never put a condom back on. Once it has been exposed to body fluids, it can be considered to be contaminated and, if you put it back on, your are subject to getting an STD.
Also, if it keeps slipping off, you may need to use a smaller size. Try one of the slim varieties.

someone mentioned about the slipping of the condom.
When someone say their condom slipped, doesnt that simply mean the condom is about to roll off the penis? Slide off the penis?
In my case, the base of the condom was firmly intact and stable. But the front section was moving. That doesnt in anyway expose anything as the entire condom is intact and gripping the base of my penis..

OK, I misunderstood what you said. You are saying your penis slipped out if her vagina, not that the condom slipped off of your penis.
So, yes, if you slip out, it is okay to slip it back it. When you change positions during intercourse, this is what you do, and you don't have to change condoms just because you changed positions.
If the front section of the condom moves becasue of your foreskin, that is ok, provided it remains firm at the base. Guys who are circumsized often will add a few drop of lubricant the the head of their penis prior to putting on a condom to provide for some extra sensitivity caused by the rubbing. Just don't add too much as it can cause the condom to slip off.
Good condoms should not have holes. They should only break if you put them on wrong or if she is not well lubricated and the tearing of friction splits it open.

Micro, you mean the head of your penis is too sensitive? This is not unusual for uncircumcised guys who haven't been pulling back the foreskin for regular cleaning of that area. There are many posts about sensitivity of the penis head in Male Anatomy and Sexual Health -- Men. Have a good look around those boards and try out the advice given there. It will be much better if you pull back your foreskin before rolling on a condom.

if u cant put it on right then dont have sex. i mean the directions are on the package c'mon all u need to is read it! so read the instuction they re put there for a reason, its that that hard to put on on

Hi Micro, I think that it is great that you're asking this problem. The one flaw that I noticed was that you squeeze out the extra air after you had the condom on, is this what you said? The proper way to ensure that air is not in the tip of the condom is this - Just as you're about to put on the condom, grap the tip of the condom (also called resevoir) and squeeze it (to get the air out) just as you're about to roll your condom on.
