do you have foreskin? Have you only recently been pulling it back when you urinate or shower?
I know i only found out when i was about 16 that you should urinate with the foreskin back (no one told me -_- and i just never knew) and at first i had similar problem i guess... it was like this really thin layer of skin or something that connected the foreskin to the head... it worried me at the time but the more i began to reveal my head it gradually went away
