you would have to go to a doctor for that, there would be some urine tests and most likely a semin sample that needs to be takken, but thats the only way i know of to find out about low sperm count.
The only way to tell is to have a semen analysis performed by a lab. As part of that they will do a sperm count and determine the number of sperm and their motility (how quickly they swim) as well as whether the sperm appear to be normal or abnormal.
Prior to producing the specimen (usually via masturbation into a container) you need to abstain from any ejaculation for 72 hours in order to obtain a proper sperm count.
The wrinkling or excess skin in the testicles is of no meaning.
