These are not 'standard' tests for a routine exam, and the only reason a doctor would test for any std is if the patient requested it, or the doctor suspected it from either symptoms he saw himself or the patient brought to his attention.
In your wife's defense, lower abdominal pain is a symptom of both gonorrhea and chlamydia, and irregular bleeding is also a symptom of gonorrhea. Since she did have both bleeding and lower pain, could be the doctor was just covering all bases as many do. And since chlamydia often shows no symptoms at all, a doctor will usually just go ahead and check for this if they're already checking for an std, which is why she was checked for both.
I assume the results were negative for both as you didn't say otherwise. I would think this wouldn't be somethig she would willingly volunteer to you if she had something to hide. If there was a reason for her to keep this from you, she would have. Because she shared it and explained it, that in itself tells me she is to be believed. She had symptoms that warranted being checked (and just for the record, if she is trying to conceive and has not, it's not unusual for a doctor to run these tests as both can interfere with fertility....they would run these tests simply to rule them out. Again, it's not uncommon for a doctor to do this).
