Shoulder Impingement/Cortisone Shot

Hi,
I was just diagnosed with shoulder impingement, and given a cortisone shot on Monday. Happy to say its starting to feel a bit better. Start PT on Monday the 5th.
How long does the cortisone shot last? Will it wear off and the pain begin again? Has anyone else had experience with this? How long did it take to heal? Mine has been sore since late December. Didn't know what was wrong, so I hoped it would get better but it wasn't, so I went to see a Dr. He said he doesn't think there's a tear but that i just irritated the rotator cuff muscles..
Any info would be appreciated.

Hi Carol, I suffer from the same problem I have had my shoulder injected about 5 times, usually every 6 months. I get 4 really good months, and slowly go down hill at the 6 month Mark I am more than ready for it too be treated again.

My orthopedic shoulder specialist gave me a cortisone shot as a diagnostic measure. It worked really well for about a month, then the pain came back.

I tried PT first. While I understand it helps the majority of the time, it didn't for me. I steadily got worse. Eventually, in addition to the pain, I lost range of movement. I couldn't move my arm behind my back at all and it really really hurt to raise it.

I ended up opting to have a subacromial decompression done. They "cleaned up" my shoulder and shaved down a bit of the underside of my acromium to give me a bit more room to help keep the inflammation down. Within months, my ROM was back to normal. I still have problems here and there, but I'm very glad I had the surgery.

I have problems with hypermobility though. I'm not sure it is the same for everyone.

Hi Carol, the PT really helped me. It strengthened my shoulder and the hope is that the injection will last longer, they really do not want to keep injecting steroids into the shoulder as in the long term I believe it can cause problems.

We'll se how the PT works, hopefully it will help. I'm sure the repeated cortisone is bad in some way, so I'm hoping to not need more. I did hear that it causes weakness in the muscles. I hope not- that was part of my reason to work out- to strengthen my upper body muscles! So far that part didn't work out LOL!

Just how many cortisone shots can the tendons in a shoulder joint survive?James

I am not sure, my rheumy said that two many can cause other problems I think it can weaken the bone itself but am not certain.

The Doctor ordered some Xrays, and reviewed them to rule out arthritis. No calcification or any other issues. He then did some range of motion tests on my shoulder by moving it around and asked some questions. He said its probably not a tear but said I did irritate the rotator cuff. He suggested the cortisone shot and PT. Said if its not better in 2 months, to come back. He wrote the diagnosis as Shoulder impingement.

Its a bit better but I still have pain when I move it certain ways. Its not waking me up at night any more at least, which is the main reason I figured I better get it checked out.

Do you have any advice?

Hi James,

It is painful when I lift my arm up from my side, palms down. The pain starts a little before my arm reaches a 90 degree angle. Also when I lean on my right arm, which I never realized that I do that often when getting in and out of bed. It also hurts to reach across my body to touch my left shoulder. I don't have much pain when reaching up to wash my hair, or when reaching straight out, however when I reach out and pull something, like a heavy door, that sometimes causes pain. I also tend to sleep on my right side, and that is still painful a bit. Since the shot, my pain is not as bad though.

Fistly Shoulder Impingement is not a true diagnosis, all it suggests is that something is being impinged, that is compressed. In theory your PT is the one who should be able to let you know the exact structure at fault. Several causes can be seen if you look at the anatomy, such as the acromion, tendons, bursae and the shoulder joint itself. Two bits of information you gave were vital, one palms down and pain by 90 degrees and equally important that placing your hand to your opposite shoulder ellicited the pain.
This suggests it is the subscapularis muscle tendon. I have found out on the net there is a new " inferior approach" for injecting but it did not explain unfortunately what it was. Try this and let us know what happens, have someone place the hand of your sore shoulder on the opposite shoulder and apply pressure and see if it becomes more painfull.

I have the exact problem with my rt shoulder. I am starting PT this week, but my doctor said he may have to go in and clean out the inflammation and smooth the bone.
