treatment options? mri results

i was in a motor vehicle accident a few mos ago and hurt my shoulder

MRI results:
partial thickness undersurface tear of supraspinatus tendon involving about 30% of the articular surface fibers. it also mentions type II acromion anatomy w/o significant enthesophyte formation and very mild degen hypertorphy of the acromioclavicular joint and slight inferior articular spurring, but no resulting subacromial space narrowing.

I dont know what this means, but any thoughts on what my treatment options are? i've done about 8 wks of PT and i'm still in a lot of pain, hurts to sleep on it, lift it overhead and to the side as well as reaching backwards.

thanks in advance for any input.
