I need advice - PLEASE

My twin brother called me last week complaining of extremely bad pain and a swollen area in his groin area. From everything he described, I assumed it was a hernia and advised him to go on to the doctor - especially since the pain level was so high. The family doc that he saw told him he needed to be transported to the hospital because she was 99% sure that it was a femoral artery clot and that it was very serious. So they rushed him to the ER. Once there, a doppler was performed and the blood clot was ruled out. They did, however, spot a mass that had a blood supply going to it near the lymph node in his groin area. So they did a couple of cat scans that showed the lymph nodes from his groin all the way to his liver were enlarged and inflamed and also his spleen was enlarged. The ER doc said it could be anything from infection to lymphoma and scheduled a biopsy for the next morning at a hematologist's office. Well, the hematologist only examined him for 5 minutes before determining that it was cat scratch fever. He gave him Keflex to take for 5 days every 6 hours and told him to take motrin/ibuprofen to get the swelling down. Well, it has been five days and my brother went in for his follow up treatment. There has been no improvement what so ever and the antibiotics have made him feel nauseous and run down. But the doctor took all of 5 more minutes to decide to continue the course of treatment we are on and come back in two weeks. Doesn't it seem like if the treatment is not working, then maybe other avenues should be explored? Does anyone have any experience with Cat Scratch Fever and does this seem like teh course the disease usually takes? Any ideas or advice will be greatly appreciated. I hate to see my brother in so much pain! Also, do you think we should get a second opinion? Please help me in any way that you can.

Re: I need advice - PLEASE

I do know that most cancers are painless. Also, most lymph nodes that are swollen are painless as well, if from cancer. So the fact that he has major swelling and so much pain is a good sign, as far as cancer is concerned.

However, it does sound like he has some sort of infection, or cat scratch fever. Did you do a search on the internet for cat scratch fever?

Re: I need advice - PLEASE

I did do a search and found that it is most common in children, is usually self limiting and is non bacterial. If it is, in fact non bacterial (I read contradicting articles on this) then why would you treat it with an antibiotic?

I work with some doctors and was reassured that it is a very painful, annoying disorder that can mimick lymphoma in a regular check-up. He said that even on strong antibiotics it can take 10 days to two weeks before you start feeling human again. So I guess time will tell.

If it is non-bacterial, then no, there would be no need for antibiotics. However, I think the doctor gave them to him just as a precaution. I think they want him to take the antibotics to see what happens because they dont want to start cutting on him just yet.

He can get a second opinion, that would be a conservative thing to do. But I would wait a few more days and see how he is doing. He could go ahead and make the appointment and then if he gets better the can cancel it if he needs to.

It still doesnt sound like cancer to me.
