Re: Can Someone Help

Hey Bucky, Compounding instructions are sually pretty specific as far as the doc orderring the mix as far as the proportion DXM to oxy, or in this case, if they are trying to make it abuse proof, the only known way to do that is to use Naltrexone and put just enough opiat antagonist in it to prevent a high when it's injected and not disrupt the anelgesic effect or send an opiate dependnet patient into withdrawal.

They had the same problem with Talwyn back in the '70's "people injecting oral pills" so they added Natrexone "Naracan" the opiate antagonist that purges opiate receptors in small enough amounts it was only activated if crushed, but it did put a cap on the effectiveness of Talwyn once you had enough opiate anatagonist in your system..

After the big media frenzy with Hillbilly heroin and all that mess, Purdue puled the 160 mg tablets from the market and tried creating it's own verion of OxyC NA, only they couldn't get the proportion right. Adding Naltrexone wasn't quite as simple and after clinical trials where the analgesic effect was lost when natreexone was added or it it caused folks to go through withdrawal. Because the antogonist action of naltrexone blocks opiate receptures from pure opiate agonists like oxycodone. It's not some special thickness in the capsule or none wouldbe released untill the 6th or 8th hour.

At the time I figured it was just a mathmatical question as to how much Naltrexone to use and still keep oxy effective and not cause withdrawal in dependent patients. Purdue Pharma gave up after spending several million on new R&D to mak an abuse proof OxyC by adding naltrexone. I wonder what your doc knows and your pharmacist knows that Purdue Pharma couldn't figure out.

Perhaps not much more if the cause of the withdrawal is the small amont of opiate anatagonist they are putting into each pill. It reall makes no sense to compound a med thats available. By creating an Oxy product with Naltrexone, they are basically producing a non FDA aproved drug and you are a guinea pig. I sure hope they know the right equation as not to put so much nalltexone in oxy as to nullify all opiate activity and cause withdrawal??

It really doesn't make sense, are you in Canada where you don't have a FDA watching every drug that makes it to market or are the doc and pharmacist related where this is a financiallly benficial relationship for both to make all your meds compounded only. If Purdue couldn't make it abuse proof and still an effective drug, I have to wonder where they got their equations, proportions of natrexone to use and how many other patients are involentary particpants in your docs own clinical trials of his magic oxy brew. In the states, this wouldn't be legal to formulate your own brew and sell it without any clinical data backing it's effectiveness and safety.

Whether it has DXM or naltrexone or both, It's a combination that has no clinical data to support his proportions and use of these meds in combination effectively or safely. I guess if your still going through withdrawal past 15 days, I would be concerned they don't have the family recipe quite figured out yet.

BY the way, the fact you know it can't be absued doesn't mean the addict will know it has naltrexone in it, and won't steel it from yur home when he reads oxycodone on the bottle. How is this one frormula compounded by one pharamacy with no clinical trial getting the word out their med can't be abused or has naltrexone in it, going to slow oxy abuse in anyone other than the patient it's compounded for. There is something very squirely about this situation.

If this stuff makes you sick and you end up in the ER , what do you tell them your taking. Doctor Mike and Jakes special home brew tonic # 47??? Take a pill to the hopital to have them analyze and figure out what the heck your taking before it kills you. How do you counter the effects. These questions are why you can't just whip anything up you want and pat folks on the head and say trust me, Old number 46 had some problems but # 47 seems to be working on my other patient?
Good luck, Dave

Re: Can Someone Help

I think one of the reasons he does not get the generic oxycontin is because he is in Canada. Here in Canada there is not a generic version of oxycontin yet. I am on oxycontin brand name and have been for a few years, it is dosed 3 times daily for me which works alot better than the 12 hour dosing that I started out on. At one point my doc decided to try me on the compounded oxycontin. There is a pharmacy in my area that specializes in all sorts of compounding and specialty items. It was when we came to an agreement that I needed an increase in my dose, he thought that it would be a good idea to try the compounded oxy because we could put the dose up by whatever we needed unlike the brand and generic. I am not 100% sure of what exactly was in it. Clear small capsule, oxycodone, ginger, and I believe dextromethmorphan was also an ingrediant. They also do another oxy that has the same ingrediants but instead of ginger, cayenne pepper is used I would think that is to try and prevent snorting. As far as the ginger one of the pharmacists said it was to prevent and help stomach soreness which is sometimes a side effect of opoids. Anyway they are prescribed as an 8 hour med, but I found that 5-6 hours would be the max that I would get for relief which made me wonder how the compounding oxy could be long acting?? I was on the compounded oxy for less than a month because I was not getting the relief that I was before on the brand, they worked great for 5-6 hours but that was it. so they did work just not long enough. I made an app. half way through the month brought the remaining meds and my doc wrote another script for my regular oxy. Oh yeah I also had a hard time trying to get my drug company to cover the compounded which I won't get into right now. I have really good coverage now my plan covers 80% which leaves me to pay only 20%, and my girlfriend has 75% coverage leaving her to pay 25%. So my plan covers 80% and her plan which I am a secondary on covers the remaining 20% so I now get 100% coverage for all my meds which is sooo awesome because the oxycontin is so expensive: for (90) 40mg tablets $245 and for (90) 10mg tablets $100, thats some pretty big savings.
