An arrythmia is any disorder, disturbance of the normal timing of heartbeats and its function. It can orginate from the sinus node (natural pacemaker) can be interference of the impulse pathway, impulse can be diverted to the wrong pathway, etc.
For perspective, to help diagnosis the QT wave should be modified with the heart rate due to interval variability and this would be designated as QTc for proper diagnosis. Depending on what EKG probe and angle a prolonged QT interval could be hypertrophic cardiomyopathy. The theory is it takes longer for an impulse to go from Q to T tracing as measured in msec. on the horizontal axis. The amplitude of the impulse as measured in mvolts, if high, could mean the increase in potential is due (possibly) to heart muscle damage, etc.
There are 12 different leads (probes) that complicates any diagnosis. At the top of the EKG report the software prints its automatic diagnosis based on the results of the test, the doctor determines the relevancy.
A 24 hour monitor will capture the impulse and provide a tracing at the time of the arrhythmia. An EKG is not very helpful for arrhythmia unless there is an event at the time, however, it can be helpful to diagnose heart muscle damage, enlarged heart, heart wall thickness and ischemia.
