Questions on swelling, meds

I have posted previously about having heart valve disease. Today I noticed some swelling above my sock top on my legs. My feet and ankles have never had swelling before that I know of. I recently saw my cardiologist for a followup echo, first visit since I got the news on the valve problems. It was reported to be "okay" which I assume meant no change. I was tried on Benazepril and Cozaar and could not tolerate as I had coughing spasms. I have been given a Pro card ia script that is away in mail order at this time. I was wondering if anyone has had any experience with this drug and what precautions may need to be taken. I know the cardiologist was concerned at my visit that my BP was low even after I had been walking the halls. He questioned me about what a normal BP is for me and seemed hesitant to give me this med, but indicated he wanted me on something to take the stress off my heart because of all the valve problems. I also am awaiting an MRI next week for a possible cervical nerve impingement either above or below a previous cervical fusion. Any wisdom on the *******ia, swelling? I was hoping to delay the start of this med pending the cervical MRI evaluation as I have so many drug reactions and allergies in my background. Thanks in advance for those who share your wisdom.

Re: Questions on swelling, meds

Hi goldyfm,

You didn't say what kind of valve problems you have, and what valves are affected. I am assuming the meds your doc gave you are beta blockers? If so, they also lower your blood pressure, which might be why he questioned your normal BP. They are used in heart valve problems to slow the heart rate down. Do you remember what he said your BPM were?

I also have valve disease and my doc declined to put me on beta blocker because my HR was within normal range.
When I saw him again a couple weeks ago he offered to go ahead and give them to me because of my PVCs. I refused, because I can control them with Magnesium supplements right now.

I don't know why your doc would want you on something that would lower your BP even more if he was concerned that it was low in his office.

The only thing I can suggest to you is read up on whatever is wrong with your valves and be as informed as you can be.

I also have swelling above my socks. I had them last year and mentioned it to my doc then, and brought it back up this time. It isn't so bad that anyone else would notice, but I do. He didn't seem concerned. I have it more in my left leg and ankle than in the right, but in the last couple months my right has begun to swell a bit at the the end of the day too. I also might add, that I have some pains in my leg and feet as well, and he asked if I had done something to strain my ankle. I don't think it would have stuck around for the whole year if that had been the case. I wonder if this is just something that we have to deal with as long as we have valve problems or a part of growing older. I haven't noticed that it gets any better after keeping off it, or keeping it elevated. Have you tried that? Did it help?

Re: Questions on swelling, meds

Mileena, I had posted in detail the results of a TEE from a few months ago. I have mild to moderate regurgitation in the mitral, pulmonic and tricuspid valves and moderate to severe regurgitation in the aortic valve. I have never noticed any swelling prior to today. One of the previous meds was a beta blocker, then cozaar which is a different classification drug and now he has ordered the pro_cardia. I have been under a lot of stress from the pain of a possible cervical rupture and am awaiting an MRI next week. The way the doctor approached the subject of my BP was that he took it several times and did not like that it seemed lower than usual, but I had only seen him once in the office and once at the hospital for the TEE. I also have other health issues, FMS for one, so I am constantly in pain but because of multiple medication allergies and bad reactions to others, it is hard to medicate me for pain. I have been going to PT for the "possible nerve impingement" but I have not had any luck controlling the pain, which I think is affecting my BP. Thanks for your input.

Re: Questions on swelling, meds

Goldy,

In my experience, doctors don't seem too concerned about the edema. I sometimes get nasty peripheral edema. When it first began, I'd have swollen feet & ankles. Some time later I started retaining fluid in my legs. Then in my abdomen as well. I feared the fluids would start accumulating in my lungs as I was also experiencing a severe shortness of breath. I asked my doctor if this could possibly happen and the answer was yes. It hasn't so far.
I was given a huge dose of diuretics (which, I have since read, are often ineffective). They seemed to help. I am told the edema could be of cardiological origin but is most likely from the medication I am on. My meds include a max dose of a calcium channel blocker, as well as a beta blocker. It is possible that your edema is a direct result of your taking multiple medications. Some meds cause us to retain sodium and water. When our plasma levels increase, fluid leaks from the smallest blood vessels - capillaries - into the surrounding tissues. I don't know of any way to keep edema from happening except to stop taking the offending medications, or reduce the dosage. Unfortunately, discontinuing the meds is not an option for some of us.

The best way to treat the swelling is by restricting your sodium intake and physical activity (if possible). Have your electrolytes checked frequently.

Good luck with your treatment and pain management!

Question Re: Questions on swelling, meds

Thanks for your comments on the swelling. I have been notified that the Pro-cardia is in the mail and I retrieved my loaned out BP monitor so I am ready to start once it comes in the mail. I was wondering if it is advantageous to take this new med in the AM, I am afraid to add to my nighttime meds as they are sedating, which I would think affects the blood pressure already. I am so hesitant about adding any new meds given my history of allergic reactions and being intolerant of a number of meds already. I just thought it may be easier to monitor the effectiveness in the daytime. Any opinions?
