You really are in a pickle. Welcome to the barrel! One other thing you might want to do is pull some articles off the internet to take to your doctors/dentists. Some on paion control and some on TMJD would both be appropriate. They are available at TMJ.org and other TMJ sites. Using the printed literature written by specialists in the field was the best thing I ever did. I helped my doctor educate himself and proved to him that I was quite serious about finding real treatment, not just drug seeking. But I do agree that darvocet-N 100 will be of little help to you. You might want to try Celebrex or Bextra or Vioxx--or the Canadian equivalent. They help me and many others also state that they get relief from them.
At any rate, I hope you find some answers that can help. I know Kaiser from the US and sympathize with you. Advocating for yopur own treatment is a great deal of work but it looks like you have no choice. I wish you the best of luck.
