Question on application

I filled out my SSD application online and today recv'd my info back that I had given them (W2, W/C wages, etc). I was looking over my application and have a question about something.....I was injured on 6-2-2007 and have not been able to work full time since then but I have been working part time from the date of injury until Sept. 1st when the doctor told me I couldn't work anymore. What should I have put for the date of disability, 6-2-2007 or 9-1-2009? I put Sept 1st because that is when I could no longer work at all...if this is wrong how do I go about changing it now???

In order to be eligible for SSDI you must be disabled for 5 full calendar months. So using the 09 date seems appropriate. In addition, if you used 07 SS would search their records and would discover you continued to work beyond 07.

Thanks for your response....so the date of disability is the date when you no longer can work at all? I have seen people on this site ask about working part time while they are waiting for SSD...how would their date of disability be determined if they are still working part time?
I must be out of work completely for 5 months before I am eligible?

I went from a full time to a part-time job, the last I worked and they deemed me disabled from the last day that I worked part-time. Just my experience.
