Help with lab results and ? for doc...please

I am still working on getting my thyroid and my body to work and feel better. My PCP a D.O. forced me to an endo because she couldn't get things regulated. She calls me difficult, then laughs and corrects her meaning. She became more frustrated than me trying to get this worked out.

Ok so here are my lab results from the endo, I started with the endo in June and she started me on 75mgs. of Armour. I had been taking synthroid and I figured out that it did not agree with me.
7/09 FT4 0.63 range 0.71 - 1.85
TSH 1.89 range 0.51 - 5.01
Total T3 0.85 range 0.76 - 1.42

8/09 FT4 0.76 ranges are the same as above
TSH 3.880.
TT3 0.75
Needless to say I had a lot of fatigue on this level. She increased my armour to 90mgs.
9/09 FT4 0.82 range 0.71 - 1.85
TSH 2.06 range 0.51 - 5.01
TT3 0.63 range 0.76 - 1.42

She kept my dosage at 90mgs Armour because her criteria is having TSH below 2.5.
10/21 last test
FT4 0.75 same ranges as above
TSH 1.59
TT3 0.69

My energy level increased some, but I was so tired and desperate for energy to get things done I have taken to drinking coffee with caffeine and when VERY desperate I cut up an old cytomel and take 1/4 of the 50mcg. tablets. However I never take one within 10 days of my scheduled blood test hoping it won't mess up the results.

I feel better on the armour, I don't however have random energy when I need it. I don't think I convert to T3. Other symptoms are weight gain again, muscle spasms, hair loss, headaches and a few others. Anyway any help you can offer as to why my T3 is so low and I still have hypo symptoms would be appreciated. I especially would like some help or suggestions as to what to ask my endo at my next appt. I see her next week and I am at a loss as to what to talk to her about. Any help would be appreciated.

You need a doc who will allow you to raise your Armour as high as you need it to go to relieve your symptoms and not use TSH as a treatment guideline. Your FT4 is only at 3% of range, which is so low I'm surprised you are functioning at all. Your doc wouldn't find you to be so "difficult" if she knew how to properly treat hypothyroidism.

The total T3 isn't a great test, the free T3 is far more sensitive and accurate for your actual T3 levels but I'm willing to bet they are still very low.

Many aren't doing well on Armour because of the reformulation last spring made it so much weaker. If you are going to stick with it then you will need some dosage increases and since you are only on 1.5 grains right now you might need to go as high as 4 to 5 grains before you feel some relief. Generally we go up 1/2 grain every two weeks and level it off around 3 grains or so then just go up 1/4 grain until symptoms relieve.

You might want to consider switching over to Nature-throid or Westhroid as an alternative to Armour. I get my Erfa Thyroid from Canada and really like it a lot.

Javalina thanks for your reply. The endo I see is suppose to be specializing in thyroid disorders. She attributes my low TT3 as being time taken or rate of absorbtion. I know that is bull because I take it the same time everyday and no food or other meds within and hour either side of taking it. I am about to run out of endo to go to, I have the same problem with all of them. I try to talk to them and they continually tell me everything looks fine. I think it might be my age, this endo keeps telling me that having a TSH too low can cause a heart attack or stroke and she won't be responsible for that. I just can't get my point across. I am 60yrs. old and certainly not a fossil. I exercise and try to eat right and take care of lots of family members, but it sure would be easier if I felt better. Thanks for your help, I hope I hear a few more opinions. I need to find a way to start a conversation with this doc that won't end up with the heart attack and stroke story.

Look up an excellent Norwegian study done a couple of years ago about how high TSH (and by "high" I mean over 1.4) increases the risk of cardiac death in women and show it to your doc. She is not working with the most current information and is stuck in the TSH dark ages. Many of us on good hormone replacement have TSH down well below one. It's the amount of thyroid hormones in the blood that's important, not some signal being put out by the pituitary. It's true that people with Graves disease (and the low TSH that goes with it) do have serious health concerns but you don't have Graves and you need good hormone replacement or your health will suffer greatly.
Also don't count on endos to be helpful here either. If your doc ignores the study, keep looking for a doc who's up on the latest about TSH and thyroid health. They are out there but you might have to look a while to find one.

If this endo doesn't know that Armour will suppress TSH to nearly 0, there is no hope you'll ever be treated optimally. I'm now 59, and my own TSH has never been higher than .02 for the entire time my MD has been treating me with Armour. It suppresses TSH; no two ways about it.

You have no energy reserve from which to draw because your FT4 is abysmally too low, as is your T3. You simply need more Armour than you're getting.

If you can find a general practitioner, internist, osteopath, or naturopath who is familiar with natural dessicated meds and who practices holistically, you have a much better chance of getting what you need than from almost any endo on the planet. Ask your local pharmacists if they will provide you with the names of some MDs whose natural thyroid scripts they fill. It's worth a shot.

I completely understand the fear of stroke in an "elderly" person. (Have to face facts, here. From a medical perspective, 60 is considered "elderly".) My own mother began thyroid treatment at around 67 years. (Synthroid, no T3 med.) She began having atrial fibrillation episodes shortly thereafter, and a massive stroke shortly after that. But I attribute it all to the ineptness of her wonky primary MD, who had made several awful treatment mistakes with both her and my dad prior to that. I believe that he either started her on too high (or too low) a dose, or he increased it too fast... Either one of which can cause heart arrhythmias. I'll never know at this point. I was trying to get her to ditch the quack, but she thought of him as a "friend". I-yi-yi.

One thing is sure... You aren't a "hard case". Your endos are just dumbos.
