If you see notes spread about and/or withdrawl from social activities by a normally socially active person, those could be signs of a brain chemistry problem, not necessarily AD.
I noted, in my mother, in the 4-5 years before her stroke and subsequent dementia diagnosis, that she took longer and longer to do normal tasks like paying bills and balancing her checkbook and also began to make notes and lists for nearly everything.
She also refused to go out more and more, just doing essential shopping. She practically had to be dragged to my wedding reception. I now know that this is abnormal and was a function of loss of oxygen and nutrients to parts of her brain.
For a younger person, firstly limit stress and verify the person's health parameters are normal. There can be many causes for cognitive decline and most are reversible.
