Sounds like allergies or ENT problems to me... I have a lymph node in my neck that stays enlarged for months through the transitional seasons. I have trouble with recurrent sinus problems. I have infections from allergies all the time that cause it. Most of the infections are really mild and I treat them with salt water. But I've always had lymph nodes in my neck and jaw swelling. As a child, I had recurrent ear infections and strep throat/tonsilitis, now it's allergies and sinusitis, all of which cause enlarged lymph nodes. To ease your mind, lymph node enlargement it totally normal and even healthy when you have an infection or allergies triggering it.
I also had bad lymph node enlargement and pain when I was having trouble with my wisdom teeth. If he has an abcess or something in his tooth or gum that comes and goes, that could cause it too.
