OGTT at 238 and Fasting Plasma at 88 ???

My Fasting Plasma Glucose Level is 88

AIC is at 6.6

OGTT 2 Hour ( 75 Grams ) is 238

My Fasting Plasma glucose ranged from 88 to 102 over the last 8 years and mostly on the lower end.

Three months back A1C was at 6.3 and FPG at 102
Now A1C is at 6.6 FPG at 88, OGTT 2 Hour 238

Am I considered Diabetic ? BTW I have elevated Cholesterol and on Vytorin. This brough TC to 138, Triglycerides to 182, HDL to 37 and LDL to 58.

My Physicial wants me to take 500 MG Glucophage twice daily.

OGTT was done only once and was not repeated.

Hi OGTT....here's some Range Numbers to help put yours in perspective:

Glucose serum fasting:
3.6 - 6.0 (60-100) normal fasting glucose
6.1 - 6.9 (101-118) impaired fasting glucose
>6.9 (118) provisional diagnosis of diabetes mellitus

HBA1C:
non-diabetic: 0.040 - 0.060 (4-6%)
optimal control: <0.070 (7%)
sub-optimal control: 0.070 - 0.084 (7-8.5%)
inadequate control: >0.084 (8.5%)

You could be pre-diabetic, given some of your lab test results. The fact that your Dr. wants to start you on Glucophage also suggests he thinks your body may need some help processing/producing insulin. You don't mention being overweight or whether or not you're "active" physically. If you are pre-diabetic/ Type2.....the first instructions are always:
1. if you're sedentary - get active
2. if you smoke - quit
3. if you don't know the meaning of eating healthy - learn to (grab a dietician to help if you can), especially if you've got some weight issues to deal with.

The good news.....following the above simple instructions can reverse or at least significantly slow down the progression of Type2 diabetes.

Can't comment on the cholesterol numbers as we work with mmol/L here in Canada.

Hope this helps shed some light...

I am 5'6" and weigh 124 lbs. I do not smoke or drink and a Lacto Vegetarian with Moderately active life style. Etnicity Indian - Eat lots of Rice Asge 42

Do I need to get re-tested for OGTT. Am I classified as Diabetic or Pre-Diabetic ?

hbrd,

Your results demostrate the fact that fasting blood glucose tells only half the story. While they are a bit high, your fasting blood sugars are not too bad. But the 2 hour OGTT reading of 238 is way to high. Your phase one insulin response is inadequate. It is really difficult to raise a non-diabetics blood sugar above 150.

Having said that, your fasting blood sugar isn't too bad. So your phase two insulin response is still functional. Deterioration of the phase 1 insulin response is the first stage of the t2 progression. And it looks like you are well on your way down this road. You don't fit into the typical t2 profile. Maybe it is simply the result of of all that rice!

Taking Glucophage sounds like a good idea. You should also cut back on the carbs. In addition to reducing blood sugars after meals, reducing rice consumption will lower your triglycerides. Also eat lots of omega 3 fats, like fish, and your HDL should go up to a healthier level.

How long does it take from Phase I delayed Insulin Response to Phase II ? Can this be delayed with Diet / Physical Activity / Glociphage ?
