Renal Tubular acidosis

Hi all,

This is my first post in this section. I have been sick for a year and a half and yesterday I saw a specialist in NY and he diagnosised me with renal tubular acidosis. So far, he is unsure how to treat it, he assured me that I can't die from it and that I have nothing to be concerned about but I just would like to be better. I have problems with my thyroid-hypothyroid. And the other symptoms are ruining my quality of life so I'd like to be better but so far he's unsure how to treat me. He says my kidneys aren't making enough bicarbonate.

My symptoms are:
Bad breath
a strong acidic smell from my boddy (people's eyes water when they are around me)
tired and lack or energy
my eyes and nose burns when the acid is at its worth
pain in my lower back

My doctor took blood work on Friday and I should get the results back on Monday but my CO2 levels are low, my chloride levels are high, my BUN is low as well. My urunalysis came back with blood in my urine yet my urine PH was alkaline. Has anyone else been treated for this? Has the treatment worked? Also, does anypne have the same symptoms I have?

Hi Crystalgray,

You too? Wow!!! I'm excited. I really didn't believe I'd run into someone else with it. You'll have to excuse me, I'm a bit hyperthyroid lately and can get excited pretty easy. I've evidently had RTA since June 8, 1992. Just got diagnosed a couple months ago. So no, it's not likely to kill you but does need to be treated or it can cause some long term damage to the kidneys and the bones and the muscles. It can also muck with your other electrolytes and such and make you feel a tad bit ill.

My gp figured it out and sent me to a nephrologist for treatment. 6 week wait for the appointment and I saw him all of 5 minutes. He gave me a lab slip and said see ya in a month. I go back on the 5th of next month. Maybe. I may not. I didn't like him at all and he wouldn't answer any questions. I'm hoping my gp will take care of it. Thing is, there's lots of info on RTA out there. Very little on the actual treatment. From what I've gathered, treatment consists of buffering the blood with bicarbonate and keeping a real careful eye on things, especially when you're sick with something. I've got tons of information on RTA and what it is and what it does. I like to research.

In the meantime, I've been drinking milk. Lot's of it. Over the years, I've found there's times my body screams for milk even though I don't really like it. As much as a quart a day. I'm also consciously hyperventilating. May not be the wisest move but I didn't have kidney stones on my ct scan back in June even though my ionized calcium level is high so I figured I may as well try it. I've brought my chloride level down 3 points, gotten my phosphrous levels into normal range, and managed to get my blood ph into normal range. What I'm doing is trying to counteract the acidosis with an alkalosis until someone can tell me what to do. My fear is the kidney damage that it seems to have caused and the desire not to loose any more kidney function. I'll inform my gp what I've been doing on tuesday when I see him.

I've noticed a funky smelling sweat for years. It washes off but I find it quite offensive. No one else has mentioned it but I think that's out of politeness. I'm a smoker so I can't tell you about the breath but I do know my pee smells odd. Not offensive. Just strong and not what you'd expect to smell. I cannot place the odor. Strange, I never did relate the eyes that feel like they got acid in em sometimes to that. I just rinse em out with cold water when it happens. Never had a problem with the nose though. I too have thyroid problems and I've always attributed the exhaustion to that. However, it was discovered my vitamin d level was quite low and I've found a definite improvement in the muscle weakness since the gp prescribed 800 iu vit d twice a day two months ago along with the fish oil caps he already had me taking for my cholesterol. My d levels have now come back up to normal. I've had bone damage and kidney damage from it cause it went undiagnosed for so long. I've got alkaline urine too. It never goes below 6. The 24 hour jug shows that I excrete almost no sodium, potassium or chloride.

Woulda got back to you sooner. Just haven't had a chance to browse the boards. Now that I know you're here, I'd really like to talk.

I will be honest and say my eyes are a little teary right now beacuse I've finally found someone I can realte too. I've been sick for so long and the doctors haven't been able to tell me much. Thank you so much for making me feel "normal". I am currently seeing a GP and a renal specialist, I have an appointment with a neph on the 25th. My symptoms are so irregular and all my tests results show is acidosis. I feel I am having problems with my kidenys yet my creatinine and BUN levels are normal. I am showing signs of renal disease yet my doctors can't get past the normal BUN levels so they keep testing me and kind of waiting for a change. I have been trying the milk thing an its help yet I urinate so frequently (at least 3-4 times an hour) and the smell is so strong that I really think I have kidney problems not to mention that the doctors keep finding blood in my urine. So far, I haven't received treatment for the acidosis. I have been taking baking soda with water yet my doctor said it wan't good for me. I feel so bad-each day its hard to get out of bed. I am taking thyroid meds as well but it doesn't seem to help I really think the acidosis and irregular thyroid are somehow related.

Saw the gp this morn. The only thing we didn't get a chance to discuss is the kidneys and the acidosis. I now have my diagnosis of pituitary resistance to thyroid hormone. I am both hyper and hypo at the same time. Some of my organs are hyper and some are hypo. There is a connection between thyroids and rta. I'll delve more into that research now that I have a definitive diagnosis on the thyroid. I've got to track down a list of the various thyroid hormone receptors for the various organs to find out which receptors are in the different parts of the kidneys. It will be interesting to find out if my tubules are hyper or hypo.

My bicarb level has gone to hell but my chloride level has returned to normal. The bicarb has gone below the measurable range I got a copy of the labs that were done at the hospital for my preadmission testing for the colonoscope and endocsope that the anesthesiologist refused to perform last monday. The gp has not seen the labs cause they didn't come in until after the appt and I haven't had a good chance to really look at them yet. Right now, both me and him are more concerned with what's causing this pain. I've got an u/s scheduled for friday to check out the spleen and the bulge in my upper left abdomen.

He's also sending me to a thyroid/pituitary specialist for treatment of the thyroid. I looked at this guys specialties. He also specializes in hypercalcemia. Hypercalcemia is sometimes caused by rta so he might have some input.

As to the kidneys, my eGFR declined from 95 to 70 between september and may. My creatinine and bun are in normal range but my creatinine rose from 0.7 to 0.9. I'm a little thing. My creatinine should be low. I stopped peeing except maybe 2 times a day starting in march. It was like that for months. I had severe edema. About a month ago, my sugar level went up into diabetic range and my thyroid hormone levels increased to hyper levels. I started peeing again. Edema's almost gone and I'm now making frequent trips to the loo.

I've been tempted to do the baking soda too. I agree with your doc though. I've had this a long and I'm afraid to muck with it without knowing exactly what I'm doing. I'll see what the neph has to say on my return visit. I may as well go back cause I ain't doing anything else and it's only 2 weeks away now. Maybe he won't be in such a rush this time.

I'll be doing a lot of research and will share anything I find. Keep in touch. It's nice having someone to share this with.
